Standard dosing for linagliptin: Adults: 5 mg orally once daily, with or without food. There is a single tablet strength (5 mg). No dose titration is required. Dose adjustments: Renal: no dosage adjustment is recommended for patients with renal impairment of any severity, including severe impairment and ESRD (exposure rises modestly in moderate/severe impairment but no adjustment is advised). Hepatic: no dose adjustment is recommended for patients with hepatic impairment. Combination caution: a lower dose of the insulin secretagogue or insulin may be needed to reduce hypoglycaemia risk, and in patients with severe renal impairment the combination with insulin was associated with a higher rate of hypoglycaemia.

Storage: Store at 20 C to 25 C (68 F to 77 F); excursions permitted to 15 C to 30 C (59 F to 86 F) - USP Controlled Room Temperature.
